If you elect to pay out-of-pocket fees for services rendered, the rates are as follows:
$160.00 (60 minute) individual assessment session
$130.00 (60 minute) individual counseling session
$ 250.00 (90 minutes) couples and family session
Phone Calls: We charge for telephone calls longer than 10 minutes. This refers to calls outside of scheduled telehealth appointments. The fees for telephone calls (self-pay rates) $130.00 per hour; a 30-minute call will be $65.00. Your insurance may or may not cover this. You are responsible for the cost if the insurance company doesn't pay.
Clients who carry insurance should remember that professional services are rendered and charged to the client and not to the insurance companies. Please be aware that not all issues/problems/conditions dealt with in therapy are covered by insurance. It is your responsibility to verify the specifics of your coverage. If you are using insurance, Liberty Counseling & Consultation will submit claims for you. You are responsible for any applicable deductibles, co-insurances and co-pays at the beginning of each session. Understand that insurance is billed as a courtesy to you and that you are responsible for full payment if the insurance company denies the claim.
Insurance: Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions. You are ultimately responsible for whatever your insurance doesn't cover.

◆ How many sessions per year does my health insurance cover?
◆ What is my financial responsibility each session?
Medical Mutual, Anthem, Anthem Blue Cross Blue Shield, Medicaid, Caresource, Molina, Paramount, Aetna, Buckeye Community Health, United Healthcare, UMR, Optum. We also accept some EAP companies and we work with Bureau of Workers Compensation.
Reduced Fee: Reduced fee services are available on a limited basis.
Payment: We accept cash, all major credit cards, health savings account cards, CashApp and PayPal. Payment is required at the beginning of the session. We require that a credit card be placed on file for all of our clients.
Cancellation Policy: If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. If there are too many cancellations, we may decide it too much of a risk to continue to provide service to you.
Fees for letters, forms and other paperwork:
If you request any letters, forms, or any other paperwork to be completed, such as FMLA or disability forms, please be advised that there is a fee for paperwork. My fee is $120.00 per hour. FMLA paperwork generally requires a minimum of 30 minutes to complete, due to the need for supporting clinical documentation. Short-term disability often takes longer to complete, and may require additional assessments beyond my regular intake evaluation. The time required to make copies, prepare and send faxes, and any other administrative business (e.g. preparing releases of information or requests for records; phone calls to lawyers or other non-clinical calls) not directly related to the provision of clinical services, will also be assessed based on a rate of $120.00, with a minimum fee of $25.00.
I will not complete any FMLA, disability, other paperwork or letters of support unless I have met with you for at least 6-8 sessions. I will also not complete any FMLA or disability paperwork if I do not believe I can support it based on what you have presented at intake and during sessions.
